In completing and submitting a 2025 registration application: 

I agree to abide by Pennant Hills Netball Club, HDNA, Netball NSW and Netball Australia’s Constitution, By Laws, Code of Behaviour, Member Protection Policy and Sports Injury Insurance Policy (available for viewing and downloading from Club and Association websites). 

I acknowledge that individual and/or team photographs and names may appear on the Pennant Hills Netball Club website, Facebook and Instagram page if consent is given during registration. Changes to this consent during the season must be emailed to the Club Secretary.

I understand that the Association/Netball NSW carries only minimal player’s insurance and that it is my responsibility to arrange adequate insurance cover according to my/my child’s needs. 

I am aware of the risks of playing netball with a pre-existing medical condition.  

I am aware of the risk of playing netball whilst pregnant.  

I give permission for the Club/Association to arrange any medical attention that may be required, including ambulance transport and I agree to pay all medical costs incurred by me/them.  

NEW REGISTRATIONS: 

I am aware that if I am/my child is a new player to the Club (19 years and under) I must present  a proof of age document (original passport or original Birth Certificate) in person to the Club Registrar on Grading Day or prior to the first game of the season.  I understand that I am/my child is unable to take to the court until this is completed.

ACTIVE KIDS REBATE:  

When registering school age children, I understand that the Active Kids rebate must be applied during registration and cannot be applied after registration has been completed and paid.

REFUNDS:  

I understand that if I withdraw my/my child’s NetSetGo/Junior Registration from Pennant Hills Netball Club after Grading Day I will not be eligible for a refund, unless withdrawal is due to extenuating circumstances.  

I understand that if I withdraw my/my child’s Cadet/Senior Registration from Pennant Hills Netball Club prior to the Club registering teams with HDNA, I will be eligible for a partial refund, provided the team is not left with less than 7 players. 

If the withdrawal is due to extenuating circumstances a full refund may be eligible. The PHNC Executive will then consider whether the whole or any part of the Club registration fee will be refunded. The Play HQ processing fee is non refundable and is excluded from any approved Club refund.

GRADING AND TEAM FORMATION (NetSetGo and Juniors only):  

I understand that on Grading Day, if my child is playing in 10-15 years, they will be trialled in the position of first preference, but not necessarily the position of second preference.  

I acknowledge and accept all decisions made by the Club’s Grading Committee on team selections. (Grading Policy is available for viewing and downloading from the Club website).

PRIVACY POLICY:  

I understand that any personal information collected is kept private and used for registration with HDNA and for contact purposes only.  

COMMITMENT:  

I understand that I am committing myself/my child to the 2025 Winter Competition of netball as arranged by the Association (HDNA).  

I acknowledge that all NetSetGo and Junior players are committed to training at least once a week as arranged by the coach of their team. Parents/carers are committed, but not limited to, participating in Club duties including Manager Roles, assisting the Executive at functions, taking an active role in Club projects and HDNA duties.

I acknowledge that Cadet/Senior players are committed to participating in Club activities and HDNA duties.  

As a Member, 18 years of age or older or as the Parent/Guardian on behalf of a Member under 18 years of age, I have read,  understand and agree to the above terms and conditions of registration with Pennant Hills Netball Club.
